Update August 29:

I was unable to speak to the Andrologist on August 27. He wasn’t at the hospital. He had an important appointment at another hospital. Therefore, I was seen, again, by the head of Urology, the same doctor who saw me 1 month ago. And it was just a routine appointment. He asked me how I was feeling, if I have improved and how my symptoms were progressing.

And he again prescribed me the muscle relaxant and the alpha-blocker. But he wants me to start reducing the dose of the pills. From now on, he told me to take the pills 1 day ON, 1 day OFF. To tell you the truth, I was already taking them 1 day ON, 1 day OFF, for 2 week. I reduced the dose of the drug when I started feeling better, 2 weeks ago. So, now, I'm going to start taking them 1 day ON, 2 days OFF...
And the Urologist asked me for my phone number to give it to the Andrologist. I am now waiting for the phone call from the Andrologist, for us to meet next week.

Yesterday, I made love to my girlfriend (Come on, I had to… It was our 6th birthday...). BUT, I didn’t ejaculate. I was afraid...
So, we started with foreplay, and I gave her some orgasms with my mouth and fingers. Then she gave me a blowjob for 5 minutes. I was always doing Reverse Kegels. My erection was a 10/10 the whole time! And I had no sign of PONR! Nothing.

Then we had sex for 15 minutes. Very carefully, only in the missionary position. Once again, I was always doing Reverse Kegels, but, during the 15 minutes, I was unable to control 7 or 8 Involuntary Kegels… But they were quite small contractions. Again, my erection was a 10/10 the whole time, with very little stimulation. And I had no sign of PONR. I gave her 4 more orgasms, but, at the end, I didn't ejaculate. She asked me to, but I didn't want to. I am afraid that the intense contractions of the pelvic floor, during orgasm, will strain the injured muscle again and that I will be back to square one. Therefore, I think I'll wait a few more weeks before trying my first ejaculation.

So, summary of achievements:
- Cure sudden Erectile Dysfunction – done.

- Cure sudden Premature Ejaculation – done.

- Regain the 0.5 inches of BPEL that I lost during Hard Flaccid - Done. I haven't measured yet, but I think I totally retrieve the size I had before…

- And I think I gained girth with Hard Flaccid. I haven't measured yet, but I noticed it and my girlfriend noticed it too. She complained that I was thicker. And, in reality, it was more difficult to penetrate her. I felt it tighter. A few weeks ago, I read a story of Hard Flaccid recovery, in which he said he had also gained girth. I thought it was probably BS, but maybe not… However, I have no idea how this happened…

- I think the bend and the twist are better. They were still slightly noticeable, but I think they have improved even more over the past week.

- Hard Flaccid, standing, is still around... I've been busier for the past few days. I have spent less time sitting and more time standing and walking. I didn’t undress my pants, to confirm, but I was aware that it was Hard Flaccid, many times. It wasn't “Hard like a rock”, like before, but it was semi-hard.
Besides, before yesterday, when I went to the doctor's appointment, I was a little anxious (Why? I don't know… xD) But the penis got harder at that moment. And I was sitting! So, guys, once again, you need to relax. It is extraordinarily important for your recovery that you have absolutely no stress.

And this is it. Let's see if I can heal myself over the next 2 months...

Interesting fact: I think I found why we feel even more relief when urinating than when we simply do Front Reverse Kegel (without urinating). HEAT. The penis is colder than our body. The urine is hotter than the penis. The smooth muscle of the penis relaxes even more and more easily with the hot urine flow, while we are doing Front Reverse Kegel when urinating.

Thank you for the post, TeoDeles :) It is very interesting, and I would like to explore it a little more.

First, everything you said in your thread is correct. However, it has nothing to do with LOT Theory :p LOT Theory has already been debunked and refuted. The TUG is just the contraction of the BC muscle, which is felt mostly in the upward stretch. The LOT is different for everyone simply because the BC muscle of each one has different development/hypertrophy/flexibility.

Second, why does a muscle strain occur? By one of two mechanisms:
1 - The muscle has been stretched beyond its limits (your case)
2 - The muscle has been forced to contract too strongly (my case)

Third, let's all take a look at this image, where we can see the muscle insertions of the IC and BC muscles:
https://www.pegym.com/forums/member...re103609-ischiocavernosus-bulbocavernosus.jpg

1 - The BC (5) will be stretched if the penis is stretched straight up. And it will be passively contracted if the penis is stretched straight down.
2 - The IC is different, because it has two muscle bundles (3 and 4). If we think about the image, we can understand that neither of the two bundles is stretched aggressively in any position (up or down). Only “semi-stretched”. However, both bundles will be passively contracted if the penis is in the intermediate position (straight out). It is for this reason that we can make the erect penis “jump” (through a Frontal Kegel) in any body position. When standing, the penis jumps upwards (bundle 4 contractions). When we are lying down, the penis "jumps downwards" (bundle 3 contractions).

Whenever I am doing a 12 ocklock stretching ... an automatic kegel is happening in the beginning of the stretch ... As long as I am holding this stretching direction is acting like a high intensity Kegel in my PF. Whenever I am stretching UPWARDS (in my case its my TUG direction) it feels like I have performed like 100 kegels

Just like when you stretch too much any other muscle (hamstrings, adductors,…). You feel defensive contractions. Your BC is the same. He has to gain flexibility little by little. And that is why stretching the penis straight up makes you more prone to muscle damage in the BC muscle. It is the direction in which the "tug of war" (see HANS Protocol) is most intense, if you do a Kegel or a Kegel occurs at the same time.

It is because of all this that I couldn't have spontaneous erections for a few weeks, after the injury (Hard Flaccid). And when I had the first erections, the penis had that downward bend. Because any erection leads to a passive stretching of the BC and IC (see image above) and muscles with little flexibility or in a post-injury contracted state cannot be easily stretched. And it is counterproductive for the healing of the acute injury.

For this reason, attempting sex or masturbation after the injury (Hard Flaccid) is highly counterproductive to the healing process. If someone has Hard Flaccid, they must do complete sexual abstinence until the body starts to have spontaneous/morning erections. It is only at that moment that the BC, the IC and the penis are prepared to have an erection. Until then, we have to give it absolute rest/very light squeezes.

If someone wants to do penile stretching straight up, they have to stretch with less intensity and gradually increase, little by little, just like when we train flexibility in any other muscle.

I truly believe that one of the unknown so far reasons of PEers that are creating an overtensed PF through PE is stretching at the TUG direction

And I think you're totally right. (except for the LOT part :p). Because stretching upwards creates the most intense “Tug of War”.

So conclusion ... (…) stretching DOWNWORDS relaxing the PF

Yes and no... No, because, in someone with Hard Flaccid, I don't recommend doing Stretching/Hanging straight down (nor any kind of stretches) in order to relax the pelvic floor. It will make the injury worse. Yes, because, in a healthy person, you are not stretching any of the muscles too aggressively (perhaps the upper bundle of the IC (4) stretches a little too much if the stretch is really aggressive). They are only semi-stretched. You are stretching them applying a comfortable stretch to all of them. That is why Involuntary Kegels in this position are rare. However, if you do a Voluntary Kegel in this position, you are more prone to strain the upper bundle (4) of the IC. Especially, if the penis is not completely flaccid. Because the IC gets more passively stretched the bigger your erection is. And that is why I strained my IC muscle, doing a Frontal Kegel, during penetration, while the penis was in a downward position.

In other words:
- You injured your BC muscle because you lifted 400 pounds from a full-stretch position (1 - The muscle has been stretched beyond its limits.)
- I injured my IC muscle because I lifted 800 pounds from a semi-stretch position. (2 - The muscle has been forced to contract too strongly)

It is because of all this that the safest position to do penile stretches is straight out. Because (1) none of the muscles (IC or BC) is fully stretched, because (2) you are stretching in the anatomical direction of all muscle fibers and because (3) all structures (ligaments, tunica albuginea, muscles,...) are being stretched together and uniformly.

Thanks for the detailed answer. I had the feeling that all of what you said was like that but I didn t know the terms and the deeper anatomy.
BUT I never said the LOT theory is correct....
The LOT theory is all about making bigger gains when you stretch the dick in the TUG direction... I never claimed it to be true...
I just used the term LOT in order to explain to people that when I am stretching in the tug direction I have a tensed PF and when I am stretching in the LOT direction my PF is relaxing.

Update September 7 - 10 weeks after injury:

Hey guys! :) I apologize for not posting an update in a while. I was waiting for the Andrologist to call me and observe me... which hasn't happened yet. He hasn't called me yet. I do not know why. I will leave the justification to your imagination...

Okay, it doesn't matter...

I am 95% healed! :D

- My erections are already 9-10/10 in all sexual positions (missionary, doggystyle, cowgirl, standing). And they last a long time, without stimulation. Minutes. In any position. I'm 16 again :biggrin1:

- Premature ejaculation is gone. 3 days ago, my girlfriend and I had sex for about 30 minutes.

- I notice that I have less Involuntary Kegels. And I continue to do Reverse Kegels always: throughout the day, during Edging and during sexual intercourse.

- Hard Flaccid is becoming something rare, even when I'm standing. Most of the time I already have a flaccid, soft and long penis.

- The clockwise rotation of the penis is much less pronounced. It is barely noticeable.

- The most pronounced symptom now is the left bend. This one is still here... It is a little less pronounced, but it is still noticeable. It's like I told you: I think the Left IC muscle lost a lot of flexibility, during the injury, due to the post-injury contracture. Now, it's a matter of gaining flexibility again. Little by little, a bit each day, trying to bend it slightly and progressively in the opposite direction, when I am erect. And maybe rest it on my right groin for as long as possible, while the penis is flaccid.

- I haven't taken any drugs in a week. Neither alpha-blocker nor muscle relaxant. I'm sick of drugs... xD Now I'm just taking the supplements.

And the most important. In the last week, I ejaculated twice (!!!):
1 - Last week, 8 days ago, I ejaculated the first time in 9 weeks. At the last appointment, the urologist asked me to try to ejaculate. So, I tried it... Yes, I was full of fear. And afraid to strain the injury again and ruin the recovery. But, I gave it a shot. So, 8 days ago, I made love to my girlfriend for about 20 minutes. We did several positions, but nothing too aggressive. And I was always doing Reverse Kegels and trying to avoid any Involuntary Kegel. And I ejaculated at the end.

The moment was a little scary. I tried to keep Reverse Kegeling through the orgasm, but it's a little difficult... The contractions were very strong. But the big problem was an intense burning sensation, in the BC muscle, during and after orgasm. It was a sharp burn-type pain. Quite intense. I don't even get to enjoy the orgasm well. I just thought "Okay, I blew it..."

The pain continued for about 2 hours after orgasm. I thought I had strained the muscle again… I sat on the couch, filled a bag with hot water, wrapped myself in two blankets, put the hot water bag on top and stayed in that position, doing Reverse Kegels, for 1 hour.

During the rest of the day, the penis was not too bad (it was moderately soft), but I felt the pelvic floor quite sore. The next day, the penis was slightly worse... It was not Hard Flaccid like a rock, but it was slightly worse than in previous days. I did many Reverse Kegels throughout the day, Hindi Squats, muscle stretching and Trigger Point massages on the pelvic floor. The next day, it was better. It was almost as soft as the days before I ejaculated.

In conclusion, I think that the day after I ejaculated, it was worse, mainly due to the loss of libido and because it was the first time I used my muscles intensely in 2 months. Not because the orgasm made the injury worse, or because it strained the muscle. I think I just gave it a more intense workout, that's all. And it recovered in the next 72 hours. However, I keep my advice, everyone: I think you should avoid orgasm for as long as you can! To make sure the muscle is really (almost) healed. And to maintain libido and to increase the likelihood of spontaneous erections.

2- I ejaculated the second time 3 days ago. A week after the first time. And this time it was more peaceful. The burning pain was milder. And, the next day, it wasn't as bad as the first time. And today, 3 days later, I am very well. My penis was soft 95% of my day.

And this is it. I think I'm about to kill the beast

HFrecovering, how have you been? :) How are things going?
You haven't said anything in a while... What treatments have you been trying? Are you getting results and improvements?

Mike_b, what about you? :) How are you doing?
How was the appointment with the Urologist? Did you discuss and define any treatment strategy for you?

Give me some feedback, guys.

Dude there's no treatments left for me to try lol... I'm not sure if you remember me telling you but I've had HF for 13 years... I've done every form of physichal therapy you can think of. I've gone above and beyond what most people with HF do to get better in terms of the ammount of physical therapy I've done aswell as working on some mental improvement.

I've been doing somthing called somatic pandiculation for about 4 months now which is essentially meditating while stretching/contracting different parts of my body. I like it, it doesn't drain me and leaves me feeling very grounded.

Fantastic! It looks very interesting! :) And it seems to focus on several important points of the recovery. It addresses the problem from several sides. Great! ;)

HFrecovering, I do not believe that your condition or the condition of anyone else with Chronic Hard Flaccid, for some years, is impossible to heal. I don't believe it is untreatable. This is not cancer. This is not a neurodegenerative disease. This is not a congenital syndrome or a genetic syndrome. This annoying condition is simply a muscle injury. It is not even a bone fracture. It is a contracture/strain/compression/cramp/injury of the pelvic floor (mostly of the Ischiocavernosus muscle). It's a damn muscle! All of my friends who workout have had and healed muscle injuries. And some of them were chronic. I've had about 5 or 6 (one of them quite long - strain in the quadriceps for 12 months) and I've treated them all.

I'm starting to think that maybe Chronic Hard Flaccid, in addition to having an intense Trigger Point of the the muscle mass, may also have a Tendonitis component (of the muscle insertion in the Ischial Tuberosity), especially in chronic cases of several years, due to the chronic inflammation created by the contracture (one of the reasons why I bet on and took strong anti-inflammatory pills during the first 7 days. And I have read some cases of patients who have benefited from them). But hey, what I'm trying to say is that I don't believe this is untreatable, regardless of the cause that caused your Hard Flaccid or everyone else's.

There have been thousands of people with chronic tendonitis, for decades, who have been able to heal them with a lot (months) of intense physical therapy. Our body is an extraordinary healing machine. We just need to give it the right stimuli, nutrients and rest.

I no longer remember where, but I read that a man healed a Hard Flaccid he had for 15 years, with 8 months of intense physical therapy… So, I do not believe that you are an alien case, which is different from everyone else.

However, yes, Hard Flaccid is a bitch... Because it has a large mental (stress) component associated. It is not the same as treating strains in the shoulder or in the quadriceps... But, about this, you are already fully aware.

I am sure that your condition is 100% treatable, if you give it a great dose of aggressive physical therapy and aggressive mental anti-stress therapy.

Do not give up! Read my whole thread again, read Hans's protocol again, read Pegasus treatment thread again and read TeoDeles treatment thread again. Very carefully. To conclude if you are really implementing all of our advice.

I read about Somatic Pandiculation, a few minutes ago. It looks like an interesting exercise program. And there is the meditation aspect. It looks great! Keep going! But implement also all the advice we gave you on PEGym. Address the problem simultaneously, from all sides. Increase the synergy of all treatment lines.

I implemented, at the same time, everything I wrote in my first post on this thread. All of it. Maybe I can list one or two things that I think were most important in my recovery, and have helped me the most, but I think they all contributed to my improvement. All were important. I think that having approached the problem from all possible sides was essential to my success in overcoming this.

Keep fighting! :)

I'm not trying to sound rude at all so please dont think i am in any way trying to sound sarcastic or arrogant but it's like I have said, i have had this 13 years, everything you are telling me to do i have done, ask any of the mods about me, some of the mods I have known for probably 10 years (this is my 2nd account) , I have done every single thing you can think of and more, nothing you are recommending to me is new or somthing that I haven't thought of, I really need to emphasise to you how much I have tried in order to get better, I literally did not stop trying things for nearlly 10 years and in the last 3 years I've kind of burned myself out with it all, however in the past when I have tried all kinds of ideas and physichal therapy I never half ass it, I always stick to somthing new for a minimum of 6 months before moving on to somthing else.

I just do not have it in me to do physical therapy anymore, I'm burned out and it has achieved nothing.

Somatic pandiulcation is interesting, i dont consider it exercise in the traditional sense, it doesn't fatigue me and leaves me feeling very good after.

For me HF wasn't triggered by an injury so its definitley not an injury in my case any way. In my opinion I have a dysfunctional nervous system that is stuck in a low level fight or flight 24/7, HF isn't my only health issue that I've been dealing with in recent years and I'm convinced that it's all down to extreme chronic stress. I am yet to find anything that can adress my anxiety/nervous system/stress levels but somatic pandiculation definitley appears to be doing somthing however it's a slow process.

Thanks for asking dr Romero. My penis Md visit was basically a pat on the back to come back later. Which is not bad. Had a interesting convo with my urologist. He looked up pegyms on his phone and said this site is awesome! That it's hard to find support groups and that uro is a area of stigma with it. Btw he knows the guys on the front page. Idk if it was cause I was close to the hospital but I was having a good.day. dr p told me he's happy I'm doing ok. He said kept excersising and was very compassionate about my story= a good guy. My brother gave me a pep talk later that day too: he said noone can tell you have this illness, you can still climb mountain s with your legs, and there was a guy in the waiting room there for a pre op appointment. Almost like he was growing a third testicle? Imagine his complications= nerve wise he is blessed to have a uro surgeon. P didn't diagnose me with much. I told him the story how my penis was getting boners like an accordion growing in segments. The lacerations was strangling my penis at one point. I sorta read his body language "edema" is what he wanted to say. Anyway he's a very busy man and next time I come back and if I haven't licked it I am going to ask him if he could operate. The good thing about him is that you can always come in earlier however he put me on the back burner for one year lol.

Some symptoms: erections are pointing up not down now. I've had a hard time staying in bed because I get pneumonia when I lay down too much, not to make fun of Davis Carter but my idk if it's suspensory wise but my angle can be kegaled up into a higher position and I can't remember if that was like that before the injury. The excruciating pain under hot water on my balls is gone. I still have trouble with mindfulness. I tried to reach out to Jeff Jefferson by text but I think he needs help too.

Anyway have a great day guys!

Thank you for your kind words, HFrecovering :)

I see... Okay, right. I have some questions :p

- Can you explain to me what was the cause of your Hard Flaccid? How, when and where did it appear? Can you associate its appearance to any specific situation (prior or during)?
- What other symptoms do you have in the context of Hard Flaccid (symptoms of pelvic floor dysfunction)?
- And, if you feel comfortable sharing here, can you tell me what are your other health problems?
(If you are not comfortable, do you think you can send me a private message?)

So, are you trying to tell me that you have a kind of Autonomic Dysfunction?...
Can you read these 5 articles carefully, please?
1 - Autonomic Dysfunction - Healthline
2 - Autonomic Dysfunction - Wikipedia
3 - Autonomic Neuropathy - WebMD
4 - Autonomic Neuropathy - Healthline
5 - Paroxysmal sympathetic hyperactivity - Wikipedia

- Do you have any more of all those symptoms listed?
- Has a doctor ever asked you to do any of the tests in the diagnostic section? If so, can you tell me the results?

Yes, indeed, he looks like a cool doctor. I'm glad you liked him :)

But, will you only see him again in a year?!... And he did not prescribe any treatment for you for the next 12 months?...

However, you seem to be improving! I'm very happy for you, Mike! :D Keep going

Thank you. A great day for you too, Mike :)
