m3232 as stated earlier, yes, we have been diagnosed with VL by a urologist. Read our corresponding threads for more info. Was it trauma or injury for me? Not that I recall. Is it diabetes or some other difficiency? Not really, blood tests were great. So what it is? I don't know. I wish I did, we all wish we did. Speculating and throwing around random numbers and "facts" will not help.

MikeCares, I did some searching but I cannot get enough information as to what physical therapy on the penis is. Do you have a link/website that describes the procedure?

pelvic physiotherapy would be the closest thing.

or something like this

Penile Rehabilitation: Is It For You?

but I don't think that would apply in this case

Therapeutic Treatment
The penis, just like any other part of the body, responds well to exercise, regular workouts and lots of activity. The importance of physical therapy treatment for penile health is a relatively new approach and field of treatment. Recent studies have supported the anecdotal evidence that a penis which is regularly used, exercised, and stimulated experiences better recovery.

In the early stages of treatment, exercises are mild and gentle. These consist of simple stretching and massaging activities to encourage the penis to fill with blood. A passive vacuum device may be used. As recovery continues, more aggressive exercises are introduced as well as stimulating the nerves which control erections.

Vibration Therapy is very new to the recovery process. By use of special vibrators, the penis is stimulated to help awaken dormant nerves and retrain existing nerves to aid in the erection process. Orgasms are encouraged fairly early on since the nerves which register sensation are just under the skin and are separate from the nerves in charge of erections. Regular stimulation through vibration therapy can not only restore erections, but also help you reclaim your sexual pleasure.

Vacuum devices are a significant part of the therapy process. Getting the penile tissue regularly engorged with blood will combat tissue damage, fibrosis, and atrophy. Daily use of a vacuum therapy regime not only helps with blood flow, it is often the first step in getting men to engage sexually with their body.

If you have experienced loss of penile mass, we have exercises and therapies designed to restore as much as possible of your former size. These treatments may require stretching devices worn several hours each day. If implemented early, these stretching exercises, along with vacuum therapy and physical therapy can often fully restore penile size. But even if you begin therapy much later, there is still reason to hope for restoring at least some of your lost length and girth.

Sexual Therapy
Regular, therapeutic masturbation, regular stimulation from a partner, and maintaining intimacy are vital in the recovery process

Note the above is aimed at rehab for guys who have had things like cancer surgery . Most venous leak is relatively minor and a simple routine with some k/rk and pe will get you past it.

It can be easy to attempt to research the cause of these types of things and misdiagnose yourself. This can be seen in many of the posts here where men have believed themselves to be suffering from venous leak- only to find that the cause wasn't organic. This is compounded by the fact that there's a lot of erroneous information surrounding the nature and treatment of VL.

As difficult as it may be, it might help if you stopped trying to diagnose yourself and either sought a Doppler (or better yet- a cavernosogram) and/or looked solely into what you can do to improve your EQ. This will require that you also focus on strengthening yourself against anxiety.

I got two doppler exams and both were inconclusive. I had a caverject injection for both of them and neither produced an erection so the exam for venous leak could not be completed.

When you say get you past it, do you mean it will fix the problem or simply increase the blood flow compared to the blood loss?

Can you make a suggestion for a routine or point me to a link, but keep in mind this is to address VL, not for any other reason.

I also read k/rk cause VL symptoms which is a bit counter-intuitive for someone with VL to perform no?

I'm sorry to hear that :( Have you looked into getting a cavernosogram?

What do you suspect caused this? Was it a gradual or sudden event?

Are you currently able to achieve erections under any means?

I suffer from VL as well, described in a thread in this section. Couple of days ago I had my second doppler (after an ineffective shockwave therapy), and the results were pretty similar to the first doppler.

Well as I did read on here, I believe a doppler test can prove that blood is not holding- which is a staple of venous leak, but what if there are other underlying causes?

I also suffer from long term anxiety due to my work but mostly my character, however I found it odd that both times I got the doppler, the injection caused nothing but slight pain and discomfort. Cialis 5mg daily works fine for the moment, but as I understand the injections should work even better? If so, why didn't they work for me at all when I was in the doctor's office?

Well I was the same as you. When I received the injection in the office I got hardly semi limp, when I left the office (where there were 3 people watching) I ended up with a rock hard erection for two hours.

Anxiety makes sense, did you get hard after the injection?

Also, with Cialis, did you try the 2.5 yet?

Indeed it feels almost futile trying to find a possible cure for something we don't know the cause of. And I have all sorts of thoughts as to what could be the cause.

Are you actively seeing a specialist?

A specialist like the one I'm seeing can anaylze everything to determine if there is an explanation.

If it's just a venous leak? Well, that's very vague. There should be a root cause.

Perhaps if left in such a blunder you should seek a sex specialist, which luckily I've gotten to in Canada.

Is it anxiety? In my situation I get heart pains from time to time, got myself checked and ECG came normal (with some arrhythmia), chest x-ray fine as well. Doctor told me I need to stop worrying so much, and that pain is likely caused by stress. I can also exercise vigorously for a long time, and that does not cause the pain. It just hits at random times. Has long term anxiety somehow weaken (and therefore damaged) the valves responsive for my erections? And IF that is the case, is it reversible, treatable or what...

This valve thing is something I know nothing (or heard nothing until on PEGYM) but the theory makes sense. Chronically high blood pressure from anxiety could probably take a toll on your body, just as it would in an older man. Anxiety can due many things, accompanied by stress, it can deteriorate your body. It even lowers your immune system. It's not just a word or a fast heart beat, as a sufferer in the past of long term anxiety as a younger person, it's a terrible condition that can cause other conditions by a breakdown in your body.

Could it be the way my penis has developed? It has a slight curvature to the left, and moving the shaft to my left thigh is much easier than to my right (the way to moves from the base). I read that slight curvature is normal, but I can't help but thinking that if my left (let's say) valve is weak, perhaps that's what I should focus on. Thinking I should even try vacuum device exercises, I mean what do I have to lose. My doctor quickly dismissed those devices, of course. He thinks they wouldn't help any more than ED drugs would.

The curved penis can sometimes be natural or Peyronie's disease- a condition which causes a curved penis which is a staple for causing "scar tissue," which is a rare cause of ED. One of the first things the doctor who performed my ultrasound was looking for was scar tissue. He said it was good there was none, but if you had the doppler test and never found any, then you should be good in that regard.

I dont' know about vaccums or anything like that, I'm not quite in that boat now. ED drugs still work. These are a last resort tool, when all else has failed.

Has anyone on this thread actually been diagnosed with VL?
Thats the more pertinent question.

If you search the term you will find plenty of people who are diagnosed with a venous leak- I'm one of them. Diagonsed by a urologist who specializes in sex.

I like your wishful thinking but I will reverse the question. Do you know of anyone who has been diagnosed with a venous leak (by means of a doppler test performed by a urologist) who has later refuted their diagnosis or overcome it, somehow?

From my non professional research, it seems the term venous leak is a vague one- there are a magnitude of possible causes.

Specifically, you should research the term "venous leak" in teenagers, or erectile dysfunction in teenagers. There is a much higher number of teenage sufferers of venous leak then adult, I am one of them.

Low T can have a devastating effect on erections. If one has chronic low T, it might be difficult to make improvements even with an excellent routine. This would be the case whether or not the low T is caused by a physical injury or some other mechanism. Low T can also contribute to anxiety.

Your particular situation is one where you're going to need to resolved deep seated conditioning issues related to your views on sex and performance. If the problem is a longstanding one- like yours- it's not enough to merely resolve anxieties- you very well may need to "re-learn" arousal clues.

well Big Al, I wish I could point it on either low t or anxiety, or both, but all I can say is that according to my blood tests, I suffer from low T and have had crippling anxiety when this started as a teenager. i also know i have a venous leak, though a vague term perhaps, i will know more in september hopefully and will make sure to post every piece of detail I get back.

No, just pain and discomfort for the next 24 hours.

Also, with Cialis, did you try the 2.5 yet?

I didn't, but at the moment with Cialis 5mg I don't feel 100%, more like at 90-95. And at times I seem to be getting softer (girlfriend confirmed) but then it comes back after a while, so I doubt 2.5mg will do any good. Seems to me I might even need a bit more (say 7.5mg?) but that's something I want to postpone until absolutely necessary.

I read somewhere that you should cycle Cialis with another ED drug. My urologist said you cannot build tolerance on cialis, I wonder is this confirmed, should I stop worrying about it?

Are you actively seeing a specialist?

A specialist like the one I'm seeing can anaylze everything to determine if there is an explanation.

No, but that's something I could look into. Might be a bit hard to find where I live, but you never know.

Indeed anxiety can be an important factor for the formation of ED, but whether or not it's the cause that's another story. Most often you hear about anxiety having a temporary effect on young people, which makes sense. But if organic ED is to be attributed to it (considering symptoms persist for more than a year) then that means it has somehow damaged parts of your body responsible for getting an erection. That is of course a bit far fetched considering the young age and all the coincidences. And then it is those parts that you will want to target if you are going to see any improvement. That's of course IF that's where the problem is.

For the moment I started doing some kegels (hold 5 seconds, release for another 5, repeat 10 times, 3 times a day), nothing to lose I suppose.

The two are not unrelated. Hopefully, you have been taking actions to counter these issues.

This statement is common, so I'll ask- how do you know that you have VL?

Probably a combo of several events. The main one being pumping with too much pressure and doing erect jelqs on the same day. Stupid I know. I posted about it a year ago. Things haven't been the same since. Then, while on viagra, I had an injury described as a minor injury to my tunica.

I can get erections but they really don't last long, and seem position dependent. My responce to viagra / cialis isn't consistent.

I still don't technically have a diagnosis of venous leak because the doppler ultrasound test could not be completed as I was not able to maintain an erection during the test.

I'd like to get a cavernosogram, but I can't get any urologist to take my situation seriously. I posted in great length about it in another thread. I just either get dismissed or told even if it is venous leak there's nothing we can do about it so why go through the risk and effort of testing for it.

well last week i had an ultrasound performed by a urologist who is trained in sex problems.

he feels that due to anxiety and low t, it could be culprit for the leak.. though we are now in the process of additional bloodwork and to see how i respond to Clomid or Clomidiphene Citrate.

Have you tried focusing solely on stamina work to improve your EQ?

No offense, but it's likely that the medical professionals you're conferring with are lazy or (as you stated) aren't taking you seriously. Unfortunately, that's a very common response. That being said, even if you have a venous leak there are measures that you can take to correct it- as long as it's not too severe (see https://www.pegym.com/forums/penile-anomalies-injuries/10071-ideas-combat-venous-leaks.html). From what you describe, whatever you have likely isn't severe.

EQ dependent on positioning is more of a function of gravity (and/or anxiety), since blood flow works more against gravity when you're in a standing or upright position- as opposed to being horizontal.

For the record, the above shouldn't be construed as medical advice.

Just a note- as per https://www.pegym.com/forums/erecti...us-leak-prevalence-predictors.html#post778164 , if you want more certainty then the cavernosogram is the way to go.

The recommended thread is very interesting and should provide you with more data pertaining to VL and various treatment options.

I am wondering if your erection is fine other than not holding an erection as long. Like do the glands engorge proprely or fully? That's what I find mostly If masturbate and let go of my erection the head drains fairly quickly yet the rest of the shaft remains hard. I am going to try and see another uroligist soon to get some tests done. I have come to the conclusion that I may have Corpus Spongoism damage (I am certain I have had this for years) hence why the glands don't enorge well and go soft quickly. Surgery seems to be the only real way to "fix" venous leakage but the results aren't that successful from what I have read and heard.

Yeah same here If I lye in bed my erection will last for a good while without touching it, or if I lye on my side the glands seem to get a bit more full as well. Yet as soon as I get up my erection dies quickly. I probably have venous leakage but I thought someone who who has a leak will loose their erection in ANY position, whether standing, sitting or lying down.

I agree with you, if you have a venous leak you can't keep it up in any position.
losing erection while standing up is related to blood pressure. Postural hypotension is main cause and it is normal I think.

Hi Al, thanks again for the link. I will read it all.

About the diagnosis, I am grateful for it. I am grateful for anything. The last urologist told me that my tesosterone, which is under 300, apparently maybe normal in an 80-year old man (though one site I saw 80yr old men have higher T on average) was normal. He also overlooked my TSH numbers, which were for thyroid.

I will ask him about the cavernosogram in September when I see him next. He wants to see how I respond to Clomid. He also ordered a lot of blood work so we will discuss that as well Sept 2nd.

Hmm anxiety this is very common and can be a cause of low T.

Well anxiety can do a lot of things, that I agree, but this is a 15-year old problem.

Stress can also increase levels of cortisol- a catabolic hormone.

Stress I can attest to, is an erection killer for me as well. Cortisol is something I noticed ordered for me on my bloodtest, so I will find out soon.

Unfortunately I suffered so long that I will always find it stressful for sex, I am always worried.

I just hope we get to the root cause of the diagnosis in September.

Until then, its a bit hard to sleep at night. Everyday, hoping for a recovery.
