Hey doc, you said you had these 2 rib like structures on the shaft side. I have very similar if not all the same symptoms as you exactly with the left twist and bend flaccid but more erect at the base. What were the ribs and did they go? I think I got them trying to fiddle with my HF penis and may have caused injury seeing as they were inflamed for sometime

I actually did this today not knowing you had posted this. He couldn't find any structural irregularities in my genitals by hand palpation. I guess that's somewhat of a relief, but it's not exactly a sophisticated method. I inquired if I should get some structural imaging done, but he said that they wouldn't be of any use in my situation. I don't know why he was so sure, I mean I was laying on the table literally for a single minute while he fondled me. Would've given me the peace of mind at least I guess.

Hi TKL. You can always ask another doctor for the exam. Or you can pay to have the exam done, at an imaging clinic. In your country, is it much more expensive if you ask to do a CT-Scan or an MRI without a prescription?

Anyways he came to the same conclusion as you Romero, by diagnosing a constantly contracting pelvic floor muscle. He prescribed me a muscle relaxant and an anti-inflammatory for the next two weeks. He also said that I should buy a heatpad or something like that to sit on top of and to avoid straining excercise.

It just seems so unbelieveable that a pelvic floor muscle strain/contraction would cause over two weeks of total ED, numbness, hard flaccid, reduced bladder sensation, and shift the angle of my flaccid 'hang' downwards. But on the other hand I do have these weird moving radiating sensations right where my BC muscle apparently is, sometimes a moving ball sensation in the perineum, inability to speed up urine flow, and lack of the 'jump' reflex you get when you are ending the urine stream, and straining to flatulate and so on.

This is all very common, TKL. They are all normal symptoms.
Hindi Squats and Reverse Kegels will be your best friends. Do them all day. As much as you can. Just avoiding overtraining.


Forgot to mention; Going to buy a heat pad and a yoga mat tomorrow.

I also was more erect at the base, Mi3an. It's also a common symptom. This phenomenon happens because it's at the base that there is a higher percentage of muscle fibers from the IC and BC muscles.

Regarding the RIBS, look at this image:

I'm going to tell you a secret that I haven't told PEGym yet... :p I had those ribs for over a year, BEFORE getting Hard Flaccid. (Yes, I was truly creating a time bomb...) And I didn't know what they were... But they didn't bother me, they disappeared almost entirely during the erection, and my erection was fantastic. So, those ribs didn't worry me. After getting Hard Flaccid, the ribs got worse. The funniest thing is that they disappeared completely after healing Hard Flaccid. Magic :biggrin1:

Hey doc, thanks for the reply. So my story:

2 weeks ago I went to pee, as I was doing so I pulled on my flaccid penis maybe a little too far. I felt a sharp pain that lasted 10 seconds and immediately knew I fucked up. Carried on peeing and went straight to my bed. I realised my penis looked different and felt different immediately (Hard flaccid). I went A&E that night just to be told there’s nothing wrong (urgh). I tried having sex the next day stupidly and knew I wasn’t getting up, my libido went too just like my erections. When I did start getting ercetions the next couple days And the minor hourglass effect disappeared, I only got erections through Constant stimulation and they were harder to maintain while standing however this got better over the 2 weeks. I should’ve abstained from touching my penis and trying to have sex with it. I just like pleasing my lovely gf lol. I’m pretty sure touching it and digging my nails in trying to find damage made me get those ribs which felt inflamed for couple days ( I took ibuprofen to help which it has). When I’m erect the ribs can’t be seen but feel like tiny cuts in the penis. My penis always had a nice normal left curve. However, after the incident I realised that at the base it leans left and twists more left. Also the veins on the left side of the shaft look more prominent flaccid and erect and during vigorous sex they’d get inflamed and hurt like the veins being blocked. What could that be?

Over the last 2 weeks I’d say my symptoms were improving (1st week I felt like crying because I was told it was in my head and thought I had peyronies lol), but then I had sex with a sustained erection that took a while to come down after ejaculation. Another thing doc, it’s only been 2 weeks but I’ve decided to give my penis a good 2 weeks rest to see how it heals and feels. My issue is that when I get an erection for long or have vigorous sex, the side that bends and twists starts to feel swollen and ache. Did you get this? Is this because of the ic muscle? Also how do you veins look now that your Hf is gone?

Ok, let's explore this :p

These are all normal symptoms. But, you should have stopped completely. I know that girlfriends need pleasure, but you need total abstinence to cure Hard Flaccid. No sex, no masturbation. But I will not criticize you, because you also didn't know the real nature of your problem. You did well to take the anti-inflammatory (I also took it for 8-9 days). If it's a recent injury, Ibuprofen helps.

This is normal. You have a strain in the left IC muscle.

The prominence of the dorsal veins is extremely common in Hard Flaccid. Your veins are working well. Nothing is blocked. When the veins leave the penis and enter the pelvic floor, they pass through the IC and BC muscles, which are contracted. This contraction/pressure causes the vein to become larger upstream (along the penis).

Mi3an, you are not an idiot. I think your effort, to give pleasure to your girlfriend, is to be praised. And I'm sure she agrees with me :p I don't think you should stop gaining stamina. But you must train it in a more intelligent way (adding Reverse Kegels, Hindi Squats, Edging, etc…)

If you have been following my thread, you know how important I think sexual abstinence is, to heal Hard Flaccid. I recommend you to stop completely (sex and masturbation) for at least a month. You are in an acute (recent) state of Hard Flaccid. Any orgasm you have now will be hampering your recovery. I stopped for almost 3 months. And I don't regret a single second. It was extraordinarily important.

If your girlfriends don't understand that you have to take this break, for your health, it is because they are not good girlfriends. My advice: reinvent your sex life during those 3 months. Why does my penis hang lower now, is it improvement?

Yes and no. It depends. Hanging lower can happen because your testicles are also hanging lower (sagging testicles, due to heat - see post 141). If your testicles are not retracted and are hanging lower, your penis has more space to hang lower. However, if your penis is also softer, then yes, you are improving. It is a sign that it was a minimal injury and the muscles are already relaxing.

Do EVERYTHING written in the first post, of this thread. Apply it all, simultaneously. Attack the beast from all sides.

I didn't get to do any tests. I quickly discovered that I had no serious penis problems. I would have liked to do an MRI scan, just to find the specific muscle injury site. But I ended up not doing it.

The million dollar question... Which doesn't have an answer.
It depends on so many factors. I healed myself in 4 months. There are people who heal in a few weeks. There are people who have had this for more than a decade. Obviously it depends on the severity of the injury. But it also depends A LOT on your motivation to apply EVERYTHING that is in my first post.

hey doc i've had this HF for almost 5 years now. I had an addiction to masturbating so was never able to abstain more than a few weeks. I would even masturbate multiple times a day with the HF because i was so depressed about it. My left testicle is extremely retracted and i have constant dull ache in the left side around my left testicle. Also have HF pretty bad. It has some slight improvement when i lay down or am sitting down. This all started from forcing an ejaculation while i beleive my pelvic floor was already somewhat tensed up, but didn't have HF and then subsequently developed HF.

My question is have i done permanent damage from continuing to masturbating with the HF for many years or can i still recover. I've currently abstained from touching my penis for about 2 months and some days i feel like there's improvement and other days it feels same as always.

Like I’ve got long flaccid now if that makes sense. Is that possible? My penis feels plumper and it takes longer for Erections or semis to go down. It does feel still hard when I stand up but looks fuller

This is someone that has nearly the same symptoms as us but he got it from bacteria or medicine. How is this possible and is his cure the same as our HF?

Also there’s a post on this forum from someone years ago I can’t seem to find. He stated that he has low testosterone due to low blood flow from a trapped artery on his left testicle. Could this happen to us? I notice that sometimes my left testicle is different than my right. I know that it’s normal for one to hang lower.

And finally not sure if this counts but I did take 4 0.25mg of finasteride with the last being taken month before my injury. I did recover my libido a week and half before my HF. But I highly doubt that’s related since my libido was normal. Thanks again and I apoligise for the extra hassle. I hope you’re really and not a troll too because you give me hope 😢

Hey everyone. A quick update on my progress. It has been about one month since I began reverse kegeling, stretching, and doing Hindi squats. After about 2 weeks I had good progress. When reverse kegeling, I felt my pelvic floor opening up more and more each day, especially while lying down on my side. I didn't masturbate or watch porn in that time. My penis felt looser and there was less tension in the pelvic floor. I got to the point where my libido was slowly returning and I had a wet dream in the middle of the night. I hate these but I haven't had one in ages. They never happened during my time with hard flaccid. So I take this as a great sign.

On the other hand, since then, my progress has reverted and it has been hard to return to this point of progress. I attribute this, firstly, to the change in my lifestyle since then. About two weeks ago, my tennis club closed for the season and I have not been playing. Normally I would place twice a week. Physical exercise is important. Secondly, I returned to work after a long break due to Covid and I sit for long hours everyday. I am pretty sure these are the reasons for my regression but I will have to test it out. Starting from the weekend I will return to exercising and will find a way to sit less.

Anyways, at least I had some good progress and it looks like there is light at the end of the tunnel.

Yes, it's possible. It's called momentary muscle relaxation.
Why are you reading my thread about Long Flaccid?... :rolleyes: Or rather, obviously you can read it. But, please, without hypochondriac thoughts... :p Read the thread just to gain knowledge. Long Flaccid is an extremely rare disease. If you have a hypertonic pelvic floor (if your muscles are tense), it means that the nerves that innervate those muscles are working properly. Simple as that. You don't have Long Flaccid. Forget about it. You have a totally normal and typical case of Hard Flaccid.

My definition of total sexual abstinence is: no sex, no masturbation, no orgasms, NO PORN, no pictures of cute girls, no edging, no attempts to get erections, no attempts to see if everything is alright, no Kegels, no caresses , no pats, no kisses. Nothing. Absolutely nothing. So, what the hell are you doing, Mi3an?... :rolleyes:

You had a muscle injury two weeks ago. How many times have you had sex after that? If you have a small injury on your biceps, doing biceps curls, and on the following days, you continue to do biceps curls, what do you think will happen to the small injury on your biceps? It gets worse, right?

So, let's suppose, you are having an erection, looking at your girlfriend. What are you going to do? You will simply adjust your pants, so that the fabric is not putting pressure on the penis, and you will do Reverse Kegels until the erection goes down. Got it? DON'T TOUCH YOUR PENIS. You will forget that you have a penis for 2 months. You will just touch it to pee and wash it gently during the shower.

My erections were very ugly at the beginning of Hard Flaccid. I could only get 40-50% of my normal erection, and the erection would go down quickly and completely if I stopped manual stimulation.
After 3 months of total sexual abstinence, I was having spontaneous 10/10 erections, which didn't go down after 3 minutes, without any kind of stimulation, except my imagination!! No hands, no porn, no pics, no girlfriend, nothing. I was studying, I thought about my girlfriend, and BOOM. I got a spontaneous 10/10 erection, that didn't go down for 3 minutes. I had the best erections of my life, after the worst injury I've ever had.

I had read that thread before. UnknownCause did not say which antibiotics he took. Therefore, I will not try to guess... And it looks like he took a very diverse combination. Could it has been an antibiotic that induced muscle spasms and, associated with his anxiety, contributed to Hard Flaccid? Maybe. Could it has been a concurrent injury, during sex or masturbation, that he didn't tell in his story? Maybe too. I dont know...

But yes, his cure is the same. In his case, we have to remove the drug that is causing muscle spasms, and start training flexibility. And, if an acute injury has occurred, it is important to give a few days/weeks of complete rest, before starting with the exercises.

No. That is bullshit. That's a correlation, without causation. Why? Because your testicles are not irrigated only by the Pudendal artery.

Blood supply to the testicles:
- Testicular artery - branch of Aorta artery- It is the main artery of the testicle. It provides the highest percentage of blood for the testicle. And it doesn't even cross the pelvic floor. It crosses your abdomen and the inguinal canal.
- Cremasteric artery - branch of the Lower Epigastric artery (branch of the External Iliac artery)
- Deferential artery - branch of the Lower Vesical artery (branch of the Internal Iliac artery)
- Pudendal artery - branch of the Internal Iliac artery - This is the artery that may be slightly affected by Hard Flaccid. And that explains why the scrotum skin is sometimes colder.

Therefore, our testicles are highly irrigated through several arteries. In addition, all these arteries communicate with each other (anastomosis) within the testicles. Even if the most important artery of the testicle (Testicular Artery) was blocked, all other arteries would carry enough blood for the testicle to survive.

The reason for his low testosterone was most likely the stress, anxiety and depression he has, due to Hard Flaccid. Which activate the Sympathetic Nervous System, which releases Cortisol and Adrenaline, which lower testosterone.

Thanks for that long and detailed reply doc as always. I’m going to take it more seriously now. I’ll follow the HANs protocol unless you think there’s other better ones out there.

I’ll let you answer the others now but I’ll just want to ask you about varicocele. I understand some men with pelvic issues get them and I know fixing our pelvic will get rid of it? Thanks again

This is someone that has nearly the same symptoms as us but he got it from bacteria or medicine. How is this possible and is his cure the same as our HF?

Also there’s a post on this forum from someone years ago I can’t seem to find. He stated that he has low testosterone due to low blood flow from a trapped artery on his left testicle. Could this happen to us? I notice that sometimes my left testicle is different than my right. I know that it’s normal for one to hang lower.

And finally not sure if this counts but I did take 4 0.25mg of finasteride with the last being taken month before my injury. I did recover my libido a week and half before my HF. But I highly doubt that’s related since my libido was normal. Thanks again and I apoligise for the extra hassle. I hope you’re really and not a troll too because you give me hope 😢

hey doc i've had this HF for almost 5 years now. I had an addiction to masturbating so was never able to abstain more than a few weeks. I would even masturbate multiple times a day with the HF because i was so depressed about it. My left testicle is extremely retracted and i have constant dull ache in the left side around my left testicle. Also have HF pretty bad. It has some slight improvement when i lay down or am sitting down. This all started from forcing an ejaculation while i beleive my pelvic floor was already somewhat tensed up, but didn't have HF and then subsequently developed HF.

My question is have i done permanent damage from continuing to masturbating with the HF for many years or can i still recover. I've currently abstained from touching my penis for about 2 months and some days i feel like there's improvement and other days it feels same as always.

Hi HelpmeHF! :) It's an honor to receive your questions on my thread. I have been wondering if I would ever have the pleasure of talking to you. Since the beginning of my Hard Flaccid, I have always liked to read your posts. Although you are one of the chronic cases of Hard Flaccid, I have always liked the mature, peaceful, calm and altruistic way you dealt with your disease. Congratulations on that :)

I will put your story here, just to help me remember it in the future, and to give more context to the rest of the members:
"- Hard Flaccid for 5 years. This all started when I had really bad OCD and I had developed a habit of masturbating and doing so with forced kegels to relieve the stress. Unfortunately during one particularly stressful time I forced myself to masturbating and I remember having to kegel hard just to maintain an erection and eventually ejaculating all of which were forced. Right after I ejaculated my penis developed the symptoms and started to lean to the left immediately. I believe I strained the muscle. I went to a urologist who told me there was nothing wrong and so I thought maybe it was all because of my OCD/mental stress, and subsequently i continued to force erections and masturbate pretending that it was all fine. This has only made things much worse.
- Left testicle fluctuates in size but is sometimes extremely small and extremely tight against my body, basically doesn't hang unless its in the mornings, but its much smaller. Feeling of discomfort around my left testicle area, the feeling is much stronger the more my testicle is stuck to my body, so it has less to do with the testicle and more to do with the tightness i think.
- Mornings flaccid penis is looser but gets super hard throughout the day.
- My flaccid penis is pulled to the left when it is in hard flaccid mode, when it is loose in the morning there is no pull. When my penis comes down from an erection it also seems to be pulled to the left. Something is definitely going on with that side.
- Loss of libido, libido is noticeably mostly when sleeping.
- I can force myself to have and maintain an erection standing but when the erection goes down, it becomes hard to get another one as if the muscle is exhausted from just maintaining an erection.
- The only positive sign i've noticed, is i did NoFap for 2 months and I started on a few occasions to have spontaneous erections again. Also sensitivity slightly improved. Problem is I would get wet dreams and the progress didn't feel linear at all and eventually i broke the nofap. So far the only thing that has helped even a little is avoiding all ejaculation/erections.
- I also remember when I was ignoring this condition and masturbating frequently usually forced of course, I would get the worst involuntary kegels at night. Luckily thats one symptom that has gone down.
- The longer this condition persists the harder it is to recover because i think your psychology gets messed up and the muscle i'm sure is further damaged from being tight for years.
- By the way i also have this weird muscle twitch around my PF/anus area that happens a few times a day whenever I am sitting. This has been ongoing for over 5 years.
Will this twitch/spasm ever go away? I had this even before the HF showed up.
- I actually had pretty bad varicoceles develop in my left testicle that showed up after I kept masturbating with HF. The less I am engaging my pelvic floor the less noticeable varicocele's have been, I think they are highly related.
- (Bumps) The actually "scar tissue" is on my the underside of my penis near the head. It appeared instantaneously when i strained my pelvic floor, which I don't think thats how scar tissue forms. I've had it randomly disappear at times when my pelvic floor feels really loose which is kinda rare, but its happened a few times.
- I can still have erections, they aren't the same as before obviously and I have a constant annoying discomfort around my left testicle that can some days is quite the distraction. Also low libido but I can almost always force an erection so I don't have ED at least.
- Last night I woke up in the middle of the night and HF was gone and immediately i got an erection that looked really healthy, no weird veins or anything. Unfortunately HF was back in the morning. This is the first time in a long time where the HF just completely disappeared. This makes me think this isn't permanent, but again why the HF disappeared that particular night i have no idea.
- After i'm done taking a sh*t on the toilet when I get up I feel my ass clench up which i think is normal, but the problem is i also have like my penis area clench up and my hard flaccid gets way worse and my dick even jumps up, it's like an involuntary kegel when i'm getting up from the toilet. This happens 90% of the time and i think it's not helping my hard flaccid situation. Is there a way to deal with this? It's extremely frustrating and this problem only started with the onset of hard flaccid.
- Literally penis is hard all the time so even when its half erect its still fully hard. It usually doesn't get fully erect either unless i'm laying down sleeping.
- I don't know if the muscle itself is damaged, but I can have really strong erections with stimulation so that suggests that maybe its not damaged...not sure I would assume a damaged muscle would be weaker but who knows.
- Now that i'm on nofap I don't have any varicoceles. It's all a result of this muscle problem thats inhibiting the blood flow. It's also inhibiting urine flow as well for me too.
- When sitting or laying down everything feels better than i stand up and hard flaccid kicks in.
- I'm sort of trying to look at this as a blessing really. I used to be kinda addicted to porn/jacking off and this is a chance to completely throw those toxic habits out the window. Porn is actually boring as hell and jacking off is a waste of time. With this condition you almost see the world how it really is.
- I'm just going to forget about my genitals, no touching, no sex, no nothing and try not to force erections of any kind, if they happen than great if not than leave it alone. Then i'm just going to move on and enjoy other things in life.
- The reason I feel like maybe time isn't helping with this muscle is it never gets to rest long enough, its constantly activated when you stand up or have an erection.
- Tried hindi squats, belly breathing, and no fap. I've done them consistently a few times a day for a month and didn't really notice any difference. Ejaculation puts me back to square one."

Ok. Although you are a chronic case, all this seems to me to be a very linear and typical Hard Flaccid story.

First of all. Why have you never tried Reverse Kegels?... In my opinion, it is the most important exercise in the treatment of Hard Flaccid. I know you did Hindi Squats for a long time, and they are also one of the most important exercises. However, if you do not add and associate Reverse Kegels, Hindi Squats become much less efficient.

This is the most important phrase in your story. The only explanation that exists in the human body for this phenomenon is a muscular contracture. The cause of your Hard Flaccid is a muscle injury/contracture. But this, you already know. I'm just confirming 100%.

That said, what would you do if your biceps, or your quadriceps, or your pectoral, or your hamstrings had a contracture? You will train flexibility. Right?
Your IC muscle has a contracture. Therefore, the approach is the same. You simply have a great lack of flexibility in your pelvic floor.

You have to bet on everything that is written in the first post of this thread. Mostly Reverse Kegels and Hindi Squats. Because these are the two main exercises that will give flexibility to your pelvic floor. And you are already well on your way, because I think you are already fully aware of how harmful masturbation and stress are, to heal this.

So, let's bet on an aggressive plan, so that you gain flexibility:
1 - Intense Reverse Kegels. HelpmeHF, it's really ALL DAY. To completely avoid any Involuntary Kegels and force the pelvic floor in the opposite direction. ALL DAY.
2 - Intense Hindi Squats. 5, 10, 15 minutes…. Whenever you can do them, throughout the day.
3 - Hindi Squats, with Reverse Kegels! For me, this is the Holy Grail. The ultimate stretch.
Just try to avoid overtraining. Increase the intensity progressively. Your ultimate (and utopian) goal is to maintain a Reverse Kegel 24 hours a day. And totally avoid all Kegel-like situations, which I'm sure you already know what they are.

Regarding masturbation and pornography, I think you already know my answer.

Your case is very linear, HelmeHF. Very common, very typical, with all the normal symptoms. It's just chronic... Because you probably had a more serious muscle injury, which you injured again and again with your masturbation habits.
Do I think it is permanent? No. Don't even think about it. Forget it. It is not permanent at all. You just need to be aggressive and constant in relaxing your muscles in the opposite direction.

Don't you know people who have had muscle injuries for several months/years, and who are able to fully recover, with physical therapy, time, rest and good nutrition? It is exactly the same here. There are milder injuries and more serious injuries. And the rate at which each body recovers depends on many factors. But what really matters is that the body is a healing machine. You just need to offer it the right materials and stimuli.

Regarding your left testicle. Bet on HEAT too. Heat, heat, heat. 24 hours a day too. This, if you also want to bet strongly on Trigger Point massages. Because the testicle doesn't bother you, when you are doing Reverse Kegels or Hindi Squats. Regarding the massages, I recommend you to do them lying on the bed, with your legs spread and covered by several blankets. To really make you sweat. The heat expands muscles and tissues. I'm sure your scrotum will be loose and the testicle will go down.

Alright I guess it doesn't matter, the improvement means i'm on the right track.

Correct. And therefore, nothing is permanent.

Basically he pursued the psyichal therapy thing for years then came to the conclusion that HF is a nervous system / stress / tension disorder and he got rid of it with meditation.

The book is kind of weird because the majority of the book is his old theory on HF and then he updated it right at the end to say that basically everything in the book should be ignored and HF should be treated as a mind body issue.

Its a shame he's not around anymore because he is by any definition the world expert on HF. Very knowledgeable and helpful guy aslong as you were on the right side of him.
