I use the cuff. when totally clamped it's like an eyelet. In my experience, loss of engorgement(if you mean erection loss during clamping) happens but you have to do kegels to keep up. for instance, perform about up to 10 kegels and with each one, press the clamp down towwards your base(pelvis).
