WHAT IS THIS AND WHY DOES IT HAPPEN
Generally this condition started to occur when more men started doing penis exercises like jelquing, stretching and vacuum pumping in order to lengthen or thicken their penis.I know there are some that do not agree with this conclusion but even in the 1 or 2 medical articles written about this it is said that all those three are the main culprits behind this-of course combined with chronic masturbation. As an evidence that over 80% of this happens because of manual penis exercises it is enough to go on a forum called PEGYM where you will find a specific subforum with more than 4k plus threads talking specifically about this condition. These guys found this YEARS before anyone on reddit even thought about this.I will go so far as to say that they were the ones that actually named this condition. This is not to say that you can't get hard flaccid by other means but the main ways are manual penis exercises and rough masturbation. Now i will explain why.
WHY ?
Generally the anterior male pelvic floor has two very important muscles that i will talk about-bulbospongiosus and ISCHIOCAVERNOSUS.Bulbospongiosus is the muscle responsible for stopping urine flow and ejaculation as it attaches to the corpus spongiosum (as the name implies) and the urethra passes through the corpus spongiosum. When you kegel the main muscle you engage is the bulbospongiosus.
The IC(ischiocavernosus) muscle on the other hand is quite different. In one of grey's anatomy editions it was called the erector muscle. Contraction of the ischiocavernosus muscle compresses the crura of the penis/clitoris, pushing the blood away from their roots into the distal parts. The surrounding veins which drain the penis/clitoris are also compressed by ischiocavernosus, restricting venous outflow. This increased pool of venous blood maintains the turgidity (distension) of the penis/clitoris, helping to maintain an erection during sexual arousal and intercourse. In males, ischiocavernosus also acts together to stabilize the penis when fully erect.I am also linking this study that says that ic muscle strength is linked to penile rigidity:https://pubmed.ncbi.nlm.nih.gov/3339760/. I am also linking you a 3D youtube representation to better understand where it is located: https://www.youtube.com/watch?v=hYJy56Q5RdI The IC fires autonomously alternating with the Bulbospongiosus muscle to create an erection. Pumping and holding blood in.Both muscles are kind of antagonists comparable to Biceps and Triceps. It's almost impossible to flex the BC and IC muscles fully at the same time.We train the BC muscle a lot. The BC is basically one muscle much easier to notice when flexed. It`s the one that when engaged consciously-THIS IS CALLED KEGEL can push blood up the CS into the glans.
Many have that muscle overtrained and hence an imbalance vs. the IC. As i said years and years of edging and masturbation massively strengthen our orgasmic muscle while weakening our erector one.During sex when the glans penis hits the vaginal walls this creates a reflex that helps activate the ischiocavernosus. As you may already noticed this does not exist with masturbation. This is why long-time masturbators have something called PIED(porn-induced erectile dysfunction)characterized by inability to hold erections. Now when you do jelq or stretch your dick you basically pull on that muscle-the ischiocavernosus. When you pull a muscle externally what happens is that something called myotatic reflex activates and pulls the muscle back. This is why i believe that stretching your penis strains or over the long term weakens that muscle
Typical symptoms of hard flaccid
The typical symptoms of hard falccid in flaccid state are well-known to people on this subreddit.HOWEVER I WILL CONCENTRATE ON THE SYMPTOMS EXHIBITED WHEN ERECT.The main symptom is one -INABILITY TO HOLD ERECTIONS WITHOUT MANUAL STIMULATION.And who is the muscle responsible for compressing the veins and stopping outflow thus keeping an erection-well if you read above you already know the answer.
The role of biofeedback in the rehabilitation of veno-occlusive erectile dysfunction Al-Helow MR, Abdul-Hady H, Fathalla MM, Zakaria MA, Hussein O, El Gahndour T - Egypt Rheumatol Rehabil-In this medical article 50% of those DIAGNOSED with venous leak recovered after pelvic floor exercises.
https://www.nature.com/articles/3900730.pdf?origin=ppub-This is another article basically saying and proving how the key role in maintaining erection is not only on the tunica albuginea or blood vessels but also the muscles-specifically ischiocavernosus.
https://pubmed.ncbi.nlm.nih.gov/8435738/-This is another article that comes to the conclusion that after pelvic floor training 75% of the people scheduled for surgery for erectile dysfunction, resolved or massively improved their condition. The 25% who did not experience this improvement either did not stick to the regimen or dropped out due to personal reasons.Sadly i can not find the full article i read a few months ago.In this abstract the information is criminally reduced. It is said that while 50%resolved another 25% had such improvement that they refused surgery. The authors also noted that the results would have been better if it was not for a large part of people dropping out.
https://pubmed.ncbi.nlm.nih.gov/15527607/-Yet another article proving what i say.
As you already noted there should be some connection to the state of our flaccid penis and the symptoms in the erect one.
Ways to releive this
Now what i will say is very important
O NOT KEGEL !!!!!What i did to help myself was training my Ischiocavernosus muscle.I came to the conclusion that my muscle is chronically tensed. My muscle is chronically tensed because i chronically weakened it through masturbation, edging and yes jelquing.Coming from the maxima :A weak muscle is a tensed muscle i decided to train my ischiocavernosus.
What i did is basically erect myself and then hang a light towel-It is very importanat to start easy and light in order to make sure you are activating the ischiocavernosus and not the bulbospongiosus muscle. After that i will try and raise the towel. If you can't do this you can try and do a negative.Just like when you can not do a pull up.What this means is that when you get yourself in an erect state you will REVERSE KEGEL and then while reverse kegelling try and gently push down with your finger. After a while you will be able to mindfully activate your muscle and counter on the downward movement exerted by your finger.
An alternative way to train this muscle is when flaccid. In order to do that -you will have to do a VERY VERY light kegel-try not to clench your anus but instead pull upwards your testicles.Do this and hold LIGHTLY for 30-40 seconds to start feel trembling in your penis and uncontrollable shaking which is going to show you how weak and chronically tensed this muscle is.
A very interesting thing i noticed that aslo supports my theory is that when you clench your ischiocavernosus muscle in a flaccid state the penis starts to look very hard fallcid like-suddenly the dorsal vein enlarges and thickens. The only diiference is that this time you have control.
I know all of this is exteremely hard so i want to link threads and posts that better explain it and helped me in trying to locate and flex this muscle:

What i noticed when doing this exercise is that i immediatley felt relieve when doing it.My penis was not so shrunk this time.I waked up on several occasions in different nights and noticed something -i had night time erections.I wrote all this to tell you that yes lower body training is important, yes anterior pelvic floor training is important. However you can heal this faster if you actually isolate the main culprit behind all of this.
I know a lot of people here speak about pelvic pain and fascias and all of this.Let me ask you something and i truly want you to answer it: chronic pelvic pain is found in up to 16% !!! of males.Nearly everyone has some sort of anterior pelvic tilt due to our modern lives. Most people have chronically weak pelvis due to years of sitting. There are members on this sub who train legs regularly.YET,yet even after all of this hard flaccid is so RARE and the majority of people with chronic pelvic pain, with weak pelvic muscles and with anterior pelvic tilt do NOT get it.WHY ?why ?why ?why?
That said i want to say that lower abs training is crucial-when i actually trained my ic muscle i noticed that i get more activation when i reverse kegel and then slightly activate the ischiocavernosus, at the same time trying not to clench my butt and kegel.I just noticed how my lower abs started to get super activated just like when doing leg raises.
I will also do a separate post in which i will upload a german manual on ways to strengthen the ic muscle.I found glute bridges helpful.I do not know how to upload it so i will make a separate post since i do not want to risk deleting all this wall of text accidentally. The manual is very interesting you just have to put it through google translator.Do the glute bridges and the second exercise .DO NOT DO THE FIRST ONES SINCE THEY LOOK LIKE AN ORDINARY KEGEL !
Edit:Here is the german manual-https://www.maennergesundheit.info/files/mg/pdf/MG-001-14_IC-Training_PDF_10-06-2014.pdf
A quick tip:if you watched the 3D youtube representation of the ischiocavernosus you would have noticed how close the gluteus muscles and the ischiocavernosus are. When you walk try and activate your glutes to feel relief with the hard flaccid.Clench your big butt muscles-BUT DO NOT CLENCH YOUR ANUS (this is more like a kegel )and try walking like that.
A thing i also found helpful was trying to cut carbs and go on the anti-fungal diet.Basically i do not know what is the role of the inflamed prostate in this.I tried going on a food regimen with coconut oil, apple cider vinegar, garlic and all those anti-fungals.I also took probiotics. You can also try this.I imagine if your prostate is inflamed it may help and relieve your condition.
