You can have a regular around the package ring, or you can add another ring to go around your dick as well. Just be careful not to make anything too tight. And be sure not to stretch the ribbon while measuring before cutting. You can make quite a bit for $30 USD.

I recommend wearing it at home for some time to make sure your glans isn't getting cold.

1. Measure the size of what you want the cockring to be using the string/ruler method or tailor's measuring tape, double it, and add 1/2 inch.
2. Fold the ribbon in half.
3. Insert the post through both sides of the ribbon, putting the rounded top at the edge of the ribbon. I prefer to do the loose ends first, followed by the folded end.
4. Put the snaps on the other end on the other side of the ribbon.

Just a little update on this. I've been using these for a while now. Been working great. A couple things though...

1. Not rigid enough. Material rolls/folds (not a big deal and still works fine). When using non-elastic fabric ribbon, the edges dig into the skin when the scrotum contracts.
2. With the contraction/relaxation cycling, the skin under the scrotum tends to pinch every now and then. Severity of pinching depends on tightness of the strap.
3. The difference between too tight and too loose is less than 1/4". If it's too loose, a ball can slip out. Too tight, then it pinches. Takes a bit of trial and error (and material) to get it right.

It's fairly easy to make a quick adjustment to the strap when needed, but takes a little practice to be discreet. I've made a couple cockrings without doubling the length and folding it in half on the short edge, but folding it in half lengthwise. It stretches out easier though and is easier to get a ball popping out. The reason for folding it at all is for a little added structure/rigidity.

I got a wider, non-elastic ribbon to go around the scrotum and attached a smaller elastic strap to go over the top of the penis. I can see why leather is used... it's thicker and the edges don't dig into the skin. The warmer weather might be better though - less contraction. I liked it otherwise. It breathes better than leather, which is one of the main reasons for going this route.

I'm going to try braided/knit elastic bands next and see how that goes. They're thicker and should get the rigidity I'm looking for. Not sure how well they'll breath though.

1/2 inch knit no-roll elastic from Walmart. 1 1/2 yards for $0.97. No need to double/fold it. Works great. Thicker, stiffer, stays in place, light pinching under the scrotum (if there's any pinching at all), breathable, cheap. I'm calling it a keeper.
