You mentioned multiple issues: phimosis, hygiene and visual: what is the primary reason you are considering circumcision?? Hygiene-unless you spend alot of time away from civilization, it's not hard to keep it clean: most guys make the mistake of washing too often with soap & water, which dries it out and causes itching and some pain, wash as you normally do when showering, but rinse frequently with water: before I went under the knife, when in public buildings I always looked for handicapped stalls that had the sinks in them. Visual: did you have a hanging foreskin before you developed phimosis? There is a type of circumcision that leaves some of the foreskin on. Now for phimosis-There are other ways of dealing with it: creams, and the device that TLCTuggger mentioned. But phimosis is something that will get your urologist to make a surgical appointment almost as fast as you can get the words out of your mouth: they love to do those kinds of $urgical procedure$, if you know what I mean....
What it all boils down to is do you want to be circumcised or not? If you do not, then there are options for treatment. If you do, remember that once it's done, it's done. You will lose a lot of nerve-endings: stimulation will be...well...not necessarily better or worse, just different. One thing you will lose, though, is that extra sensation of your foreskin sliding back & forth over your glans, which was nice.
One thing you do need to know, adult circumcision is not just snip-tuck & you're done, if you get the chance, try to web-sleuth some circumcision surgery pics, you'll see what it entails, it's not for the faint-of-heart. Note-the post-op healing is NOT fun. Remember that you are cutting off a part of your body rich in blood vessels and nerve-endings, A LOT of nerve endings. It is going to hurt.
Circumcisions DO get botched on occasion, yeouch!
I had it done as an adult, you can read about it here:

It was my own choice and I must say that it worked out well. But that was me, everyone is different. I'm not for or against, circumcision, its a matter of personal preference. But like many folks on this forum, I just want you to be INFORMED.
Whatever you decide, good Luck!
