Do extenders work in a similar fashion as manual stretches? Or are they two very different methods of gaining length?

Actually they do and don't. The way the extender is build is that the main anchor support is the pelvic bone which is essence will keep pressure on the ligaments just underneath, but also keeps some tension on the shaft in general where the anchor's point is just underneath the glans. The reason it takes longer four-six hours a day, it because the tension is just enough for the pulling but not enough to make the noose slip off.
Manually, you have more control on how you stretch and where your anchor points may be. My stretches are similar to the extender's which I believe assist in ligaments and shaft length because I grip at both area and pull first at the base then use the upper hand to make the difference.

In my honest opinion, manual will give your more control and probably faster gains.

Yes and yes.

Both work by putting outward pressure on the ligaments and tissue, creating a need for additional growth.

However, stretches are short timed, high tension, and done in a focused manner. Extenders are long timed, less tension, and applied in a gradual fashion.

Why is it commonly agreed that extenders give permanent gains?

in my opinion both are effective!with extenders you have 'the time',with the manual stretching you have 'the force',and it is better maybe for gains from the ligaments...i think extenders are better to pull out inner penis that why the gains are permenent (but you can do this with manual also )!i think the gains of both depends from the persons penis and from the intensity who use !

Gains are both from tunica/smooth muscle stretching and stretching the ligaments. Ligaments start from the pubic bone and run the entire length of your shaft. You should know that the process by which extenders work is different than manual stretching or hanging. The tissue is not extended to the point of creating tears while stretching. The penis in an extended state, has more gaps in it than the normal unextended flaccid penis. The body over time, starts treating those gaps as a part of your body trying to fill them with new cells to reach equilibrium. It will be much more difficult to gain through this after your initial gains because the new tissue is more rigid and less elastic and there won't be enough gaps to be filled in the future and thus create gains.

Extenders are one of the few, quite possibly only, device that has medical backing for length gains. Originally tested as a cure for Peyronie's, research showed that an increase in length was a "side effect". Of course, through user trial and testing, many have confirmed that it indeed does contribute to gains in length, and even girth.

Think of stretching as lifting weights, and extending as wearing ankle/wrist weights as you go about your day. Over time, your body will have to adapt for the added stress put on it.
