Yeah you don’t wont to instill unnecessary fear but in this case its warranted and the std I personally would fear the most is syphillis its really nasty and I think it could give the symptoms OP describes.

The "brown shapes" he mentioned had me thinking it might be syphilis as well, but the spots are characteristic of secondary syphilis and express themselves on the hands and feet. The dry skin may be a result of his attempts at cleaning, but if the dry skin is associated with a rash then it could be a symptom of syphilis.

He could very well have contracted warts (re: brown shapes), though.

The other symptoms are in line with the description of thrush.
